Cognitive Health Initiatives
Three Pillars Senior Living Communities committed to Cognitive Health Initiatives
At Three Pillars Senior Living Communities, our commitment to cognitive health is central to our strategic growth. We are dedicated to developing and implementing innovative initiatives - on our campus and in the broader community. For more details about our cognitive health programs, please reach out to Jen Graf, Senior Director of Cognitive Health, at jgraf@threepillars.org or 262.965.7133.
Three Pillars offers a continuum of care for people with dementia encompassing a range of residential services designed to meet evolving needs as conditions progress. This spectrum begins with supportive environments like assisted living facilities and memory care units, and progresses to skilled nursing, which offers 24-hour medical supervision and tailored therapeutic interventions. This continuum ensures individuals receive appropriate, compassionate care at each stage of their journey, enhancing quality of life and supporting families throughout the process.
Our comprehensive programs are designed to support individuals across the entire cognitive health spectrum while focusing on prevention, risk reduction, and personalized interventions. From optimal brain health or normal age-related memory loss to mild cognitive impairment and dementia, our mission is to enhance well-being and quality of life for all. Resources include engaging brain health programming, dementia education and training, community speakers, social events, and a lending library
Our cognitive health support programs are designed to empower and support individuals across the full cognitive health continuum, from those seeking to maintain their brain health to those living with cognitive changes and the people who support them. These programs offer information and referrals, support groups, online education and support, educational materials and cognitive health and memory screenings
Passions and Preferences
Right now, you are the expert on your own life. You know exactly how you like your eggs cooked, which music lifts your mood, what topics of conversation engage you, and what routines make you feel at home. This document is designed to capture those nuances in your own words while you can express them for yourself.
Most of us understand the importance of planning for the "big" things, such as medical directives or end-of-life wishes. While those decisions are vital, we often overlook the small, everyday preferences that actually make up the fabric of a "normal" day. By documenting these daily rhythms now, you ensure that even if big changes occur, your life continues to feel familiar, personal, and uniquely yours.
